While we accept most insurance plans, we highly recommend that you check with your insurance company as to whether the doctor is on your plan.
As a courtesy, an insurance claim will be submitted for your visit. In order for this to be done, you must present your current insurance card at each visit. If you do not have your insurance card, you will be expected to pay for your visit in full, or you may reschedule your appointment.
Co-pays are to be paid at the time of service. Co-pays not paid at time of service will be subject to a processing fee. For your convenience, we accept cash, checks, MasterCard, Visa and Discover.

Our cancellation policy
Please give us 24 hours’ notice if you must cancel a pre-scheduled appointment. Failure to do so may result in your being billed for a no-show appointment. This charge will not be covered by your insurance company, and you will be held responsible for payment.
